This study compared Korean adolescents' drug use and exposure experiences using the 2025 Adolescent Drug Awareness Survey (commissioned by the Ministry of Food and Drug Safety; 2,000 adolescents aged 14–18; online self-administered) alongside recent national surveys, including the Pilot Survey for the Adolescent Drug Use Survey, the 21st Korea Youth Risk Behavior Survey, the Survey on Media Use and Harmful Environments of Youth, the Out-of-School Youth Survey, the At-Risk Youth Support Service Users Survey, and the Youth Mental Health Survey. Given heterogeneity in drug definitions, time frames, and target populations, datasets were classified as directly comparable, limitedly comparable, or auxiliary. Lifetime drug use was 1.1% in the 2025 survey, similar to 1.3% lifetime illicit drug use among adolescents and emerging adults aged 14–24 in the pilot survey and 1.3% habitual or intentional drug use in the youth risk behavior survey. Notably, exposure to peer drug users (3.4%) and online searching for drug purchase methods via SNS or the Internet (3.3%) exceeded direct use, and future intention to use drugs averaged 5.37 out of 100. Among out-of-school youth, overall use was 1.0%, but rates rose sharply for adolescents in juvenile detention (8.7%), short-term shelters (4.5%), and delinquency-related subgroups (2.8%), revealing a dual structure between general and high-risk populations. Findings suggest that adolescent drug prevention should expand beyond direct-use indicators to include early-risk indicators such as peer exposure and online purchase searching, combining universal prevention with institution-based targeted prevention and mental health linkage for at-risk youth.
